I hereby certify that all entries on this application and attachments are true and complete, and I agree and understand that any understand that any falsification of information herein, regardless of time of discovery, may cause forfeiture on my part of any contract with Touch of Clean. I understand that all information on this application is subject to verification and I consent to criminal history background checks. I also consent that you may contact references, former employers and educational institution listed regarding this application. I further authorized Touch of Clean to rely upon and use, as it sees fit, any information received from such contacts. Information contained on this application may be disseminated to other agencies, nongovernmental organization or systems on a need-to-know basis for good cause shown as determined by any officers or designee of Touch of Clean. Touch of Clean is a drug free company. All employees are and will be subject to random drug and alcohol testing in accordance with state and federal regulation. Touch of Clean or any of these agents may require verification and will investigate any employees driving record as it may pertain to use of company vehicles. I understand that I am not an employee therefore, I am working as independent contractor (subcontractor) and not subject to any benefits directly paid by Touch of Clean or any workman’s compensation coverage. I understand that I will provide workers compensation coverage that I will acquire myself. All insurance and benefits are the responsibility of the independent contractor. The undersigned also acknowledges that they are responsible for all record keeping regarding their income, expenses and other important tax information. If the independent contractor receives compensation at a rate or above the amount set by the Federal Internal Revenue Services, they will be sent all appropriated forms and required notification and paperwork will be filed with the Federal Internal Revenue Service. I understand that false or misleading information in any application or interview may result in my release.
It is understood and agreed that the first ninety days of employment shall constitute a probationary period during which period the Employer may, in its absolute discretion, terminate the Employee's employment, for any reason without notice or cause.
